UNITE COURSE ADDITION/CANCELLATION FORM
Use this form ONLY if you have already completed the regular online Registration Form to enroll in courses offered by UNITE. This form is used ONLY to modify enrollment for the semester for which you have already completed the regular, longer form.
Complete this form and click the "Submit" button at the bottom of the page.
Please fill out the entire form. Required items on this form are marked (*); they will be used for identification, to determine admission, and to reference your University academic record.
- When you submit this form, UNITE staff receives your request via email and then processes it
- Please allow up to one business day for processing
- You will receive an email confirmation once your registration has been processed
- Check the refund schedule before canceling courses; you are responsile for tuition and fees if you do no not cancel during the 100% refund period
- Financial Aid recipients need to be particularly careful with renrollment in order to maintain eligibility
